Care Forward is a locally owned and operated organisation, specialising in health and homecare services designed to support the independence of older Tasmanians.

For 25 years, we’ve been providing care and support to people in their homes across Tasmania. Our dedicated and passionate team includes nurses, allied health professionals, care partners and home independence support staff – all working to support and restore independence, health and wellbeing to thousands of Tasmanians across the State.

Key Purpose

To provide in-home support to Care Forward consumers using a strengths-based approach which supports and promotes independence and active community participation.

The primary aim is to enhance the client’s general independence and wellbeing which will assist in preventing unnecessary reliance on support services, admission to hospital or residential care.

Location of Role

This role is available state-wide, based from one of our four office locations in Rosny Park, Launceston, Devonport or Burnie and performed in the consumer’s home.

Salary Range

In accordance with the Social, Community, Home Care and Disability Services Award
Home Care Employee Aged Care Level 1 or 2 depending on level of experience.

Core Responsibilities

  • Provide support with basic household tasks such as cleaning, bed making, dishwashing, clothes washing, ironing, hanging clothes out, vacuuming, cleaning bathrooms.
  • Ensure consumer homes are kept in a clean, hygienic, and homely condition according to the individual’s needs and wishes.
  • Provide assistance with meal preparation and unaccompanied shopping.
  • Use a strength-based approach to assist consumers to achieve goals and remain living independently and autonomously within the community.
  • Monitor client’s progress, and update consumer notes in the home record
  • Carry out basic health and safety/risk assessments in the consumer’s home, where appropriate.
  • Maintain client privacy, confidentiality, dignity, choice, and control at all times and advise your Team Leader immediately of any concerns e.g. safety issues, neglect, elder abuse, change in health status etc.
  • Respect the client’s individuality, equality, and diversity
  • Attend staff meetings as required.
  • Complete all mandatory training as required.
  • Demonstrate a commitment to equity and inclusion across race, gender, sexuality, age, religion, identity and experience and celebrate diversity of culture.

Essential Requirements

  • Full drivers licence.
  • Registered and insured vehicle.
  • Current First Aid certificate.
  • Current CPR certificate.
  • COVID-19 Vaccination x 3 doses.
  • Two professional referees, preferably from the most recent employment or educational institution.
  • National Police Certificate obtained within the last three (3) months.
