You will do
- Use a wellness approach to help clients achieve independence in their everyday tasks.
- Evaluate clients' homes and workplaces to identify improvements for their health needs.
- Conduct occupational therapy intake assessments, planning, and service delivery.
- Develop goal-oriented care plans using a wellness approach with your clients.
- Work with a multidisciplinary team to deliver programs and ensure the best outcomes for clients.
- Monitor and review client progress via phone and home visits
- Recommend and facilitate equipment and aids appropriate to individual client needs
- Participate in developing and maintaining partnerships and implementing new programs
- Contribute to stakeholder engagement and promotion of company programs
You need to have
- A recognised qualification and registration with AHPRA as an accredited occupational therapist
- Knowledge of risk management, workplace health and safety, and continuous quality improvement processes
- Great interpersonal and communication skills for client interactions and teamwork
- Self-motivation, accountability, professionalism, and reliability
- Resourceful in problem-solving and challenging situations
- A commitment to continuous learning
- A current driver's licence and access to a reliable vehicle
- Up-to-date vaccinations against COVID-19.
Ideally, you also have
- Experience in providing evidence-based clinical assessment and planning interventions for clients with chronic or complex conditions
- A good understanding of community-based service provision
