Social Worker

Join the Care Forward team as a social worker and make a real difference in older people's and their families' lives. You'll work collaboratively with clients to achieve person-centred health and well-being outcomes, providing holistic support and care considering the complexities involved. You'll have the opportunity to develop culturally appropriate service delivery models and work as part of a multidisciplinary team, ensuring the best services to meet the needs of older people. You'll also have the chance to advocate for service provision in areas of unmet need and develop innovative partnership models of service delivery. You will do

  • Work with your clients to improve their health and well-being using the most suitable support systems.
  • Ensure appropriate and holistic services are provided to meet the multidimensional needs of older people and their support networks.
  • The ability to review and document client progress while on the program via regular phone contact, consultation appointments, or home visits
  • Help to remove barriers affecting clients' ability to engage in their community.
  • Develop culturally appropriate models of service delivery.
  • Advocate for service provision in areas of unmet need.
  • Assist clients with adjustment to life changes and other transitions.

You need to have

  • A degree in Social Work and a membership with the Australian Association of Social Workers (AASW) or International Federation of Social Workers (IASW)
  • Clinical and practical social work knowledge
  • Effective communication and counselling skills to support client needs, motivation, and ability to engage in treatment.
  • Strong interpersonal and communication skills enabling effective interaction with clients and families, team members, health professionals, and stakeholders
  • Experience providing evidence-based assessment and planning for clients, focused on reablement and independence using a wellness and restorative approach.
  • The ability to work autonomously and as a member of a multidisciplinary team
  • A current driver's licence and access to a reliable vehicle
  • Up-to-date vaccinations against COVID-19.

Ideally, you also have

  • Experience facilitating education for clients, community groups, or organisations.
  • Comprehensive understanding of community-based service provision
  • An understanding of risk management, health and safety, and continuous quality improvement processes
  • The ability to evaluate and modify the effectiveness of social work programs/interventions
