Support Worker

Join Care Forward as a support worker and provide in-home client support using a strengths-based approach, promoting independence and community participation. You will do

  • Provide in-home support to clients using a strengths-based approach to promote independence and active community participation.
  • Deliver restorative-focused home support programs by providing support and guidance to clients and their carers, including personal care, cooking, cleaning, laundry, and transport.
  • Monitor and document client progress and attend multi-disciplinary meetings to provide feedback.
  • Conduct basic risk assessments in clients' homes and advise relevant team leaders of any issues.
  • Maintain client privacy, confidentiality, dignity, choice, and control at all times, and respect the client's individuality, equality, and diversity.

You need to have

  • Experience in supporting clients to achieve agreed goals and outcomes.
  • Good communication and interpersonal skills for relationship-building and record-keeping purposes
  • Ability to work with limited supervision in diverse community settings
  • A current driver's licence and access to a reliable vehicle
  • Up-to-date vaccinations against COVID-19.

Ideally, you also have

  • Experience providing domestic, shopping, respite and personal care, plus social support and transport assistance.
  • An understanding of the wellness and reablement approach
  • The ability to read and comprehend health and reablement plans and document effectively.
  • A strong understanding of risk management, health and safety, and continuous quality improvement
